Shockwave Therapy & Botox Pelvic Floor

Advanced treatments for pelvic pain and sexual health - when standard approaches aren't enough.

For many women, addressing the root cause of pelvic pain or sexual dysfunction requires more than medication alone. Shockwave therapy and Botox pelvic floor injections are evidence-supported, minimally invasive options that target specific physiologic mechanisms - improving blood flow, breaking the pain-tension cycle, and creating the conditions for deeper recovery.

Neither of these treatments is a standalone cure. They are tools used as part of a comprehensive, individualized treatment plan - precisely selected based on your specific diagnosis and what standard treatments have and haven't accomplished.

Low-Intensity Shockwave Therapy

Shockwave therapy uses low-intensity acoustic waves to stimulate tissue healing from within. The waves trigger the release of growth factors, promote new blood vessel formation, reduce local inflammation, and activate the body's own regenerative mechanisms - without surgery or downtime.

Used for
Sexual arousal & orgasm

Improves clitoral and vaginal blood flow, enhancing arousal response and sensitivity - particularly useful in cases where poor tissue perfusion is a contributing factor.

Used for
Chronic pelvic pain

Reduces myofascial pain, nerve hypersensitivity, and tissue fibrosis in the pelvic region - addressing the structural contributors to chronic pain.

Used for
Vestibular tissue health

Can improve tissue quality and sensitivity in patients with hormonally related vestibular atrophy or post-treatment recovery.

Procedure
In-office, no downtime

20-40 minute sessions. Mild sensation during treatment. Typically 6-12 sessions for full therapeutic effect. Return to normal activity immediately.

Botox (Botulinum Toxin) Pelvic Floor Injections

When pelvic floor muscles are chronically overactive - causing pain with sex, pelvic pressure, urinary urgency, or vaginismus - physical therapy is first-line. But when PT alone hasn't achieved adequate muscle relaxation, Botox injections into the pelvic floor muscles provide targeted, temporary neuromuscular relaxation.

Botulinum toxin temporarily blocks the nerve-muscle signaling that sustains chronic contraction. The muscles relax - typically for 3-6 months - breaking the pain-tension cycle and allowing PT to make deeper, more lasting progress. For many women, one to three treatment cycles is sufficient to achieve durable improvement.

Conditions treated with pelvic floor Botox include vaginismus, hypertonic pelvic floor dysfunction, persistent vestibulodynia with significant muscle component, and chronic pelvic pain with documented pelvic floor hypertonicity.

Comfort is a priority for all in-office procedures. Dr. Toubi offers nerve blocks and nitrous oxide (laughing gas) for Botox pelvic floor injections, shockwave treatment, and any other procedure performed in the office. Pain management is part of the procedure, not an afterthought.

Frequently asked questions

How does shockwave therapy work for sexual health?
Low-intensity shockwave therapy delivers acoustic wave energy to targeted pelvic tissue, stimulating new blood vessel formation, reducing inflammation, and promoting tissue remodeling. In sexual health applications, these effects improve clitoral and vaginal blood flow, enhance arousal and lubrication response, and reduce chronic pelvic pain related to myofascial or neurovascular causes.
Is shockwave therapy painful?
Most patients describe a mild tingling or pressure sensation during treatment, not pain. Topical anesthetic can be applied beforehand. Sessions are 20-40 minutes, require no downtime, and patients return to normal activities immediately. A course of multiple sessions is standard for optimal outcomes.
What does Botox do when injected into the pelvic floor?
Botulinum toxin temporarily blocks nerve signals that sustain overactive pelvic floor muscle contraction. This produces significant muscle relaxation lasting 3-6 months - long enough to interrupt the pain-tension cycle and allow pelvic floor physical therapy to achieve deeper, more lasting release.
How many Botox treatments will I need?
Most patients benefit from one to three treatment cycles spaced 3-6 months apart. The goal is to use the relaxation window to progress with physical therapy to sustainable improvement - not indefinite Botox dependency. Some patients with severe dysfunction require ongoing maintenance.
Can shockwave therapy improve arousal and orgasm?
Research supports shockwave therapy for female sexual arousal disorder, particularly for cases related to poor clitoral blood flow and tissue health. Improvements in arousal, lubrication, and orgasmic intensity have been reported. It is most effective as part of a comprehensive approach that also addresses hormonal, psychological, and pelvic floor factors.
