Accurate, Comfortable, and Comprehensive

A tight pelvic floor isn't something to strengthen - it needs to be released. Hypertonic pelvic floor dysfunction is real, common, and very treatable.

Most people have heard "do your Kegels." But pelvic floor dysfunction is not always about weakness. For many women, especially those with painful intercourse, urinary urgency, or chronic pelvic pain, the problem is the exact opposite: muscles that are chronically tight, overactive, and unable to fully relax.

The pelvic floor needs to both contract AND relax. When it loses the ability to relax fully, every function it's involved in - sex, urination, bowel movements - can become painful. Strengthening a hyperactive muscle makes it worse, not better.

Why this happens - the pain-tension cycle

Pelvic floor hypertonicity rarely develops out of nowhere. Common triggers include a protective response to vulvar pain (the body braces in anticipation of pain, and this bracing becomes habitual), pelvic or abdominal surgery, childbirth, endometriosis, chronic UTIs, hormonal changes, and stress. Once the muscles are chronically tight, they restrict their own blood flow and accumulate lactic acid - creating a self-perpetuating pain cycle even if the original trigger has resolved.

This is why treating only the "original" cause without addressing the pelvic floor itself often leads to incomplete recovery.

Symptoms - more than just painful sex

  • Pain during or after intercourse
  • Burning or aching at the vaginal opening
  • Pelvic heaviness or pressure
  • Urinary urgency or frequency
  • Incomplete bladder emptying
  • Constipation or rectal discomfort
  • Lower back or hip pain
  • Tailbone pain, especially with sitting

Treatment

  • Pelvic floor physical therapy - the cornerstone of treatment. A specialized PT performs internal examination to identify trigger points, performs manual myofascial release, guides biofeedback training, and builds a personalized home exercise program focused on relaxation and coordination.
  • Vaginal dilators - used at home to progressively retrain pelvic floor response to penetration and maintain gains from PT.
  • Vaginal or rectal muscle relaxant suppositories - medications like diazepam can be applied locally to directly reduce muscle tone.
  • Botox (botulinum toxin) injections - for patients who haven't achieved sufficient improvement with PT, Botox injected into the pelvic floor muscles provides targeted neuromuscular relaxation lasting several months, during which PT can make deeper progress.
  • Addressing underlying causes - hormonal deficiency, vulvar skin conditions, or vestibular pain that are driving the protective muscle response must also be treated for lasting results.

Botox injections are performed with nerve blocks and nitrous oxide available. Dr. Toubi makes patient comfort a priority for every in-office procedure - not an optional add-on.

Frequently asked questions

What is hypertonic pelvic floor dysfunction?
Hypertonic pelvic floor dysfunction occurs when the pelvic floor muscles are chronically tight and overactive. These muscles are supposed to contract and relax. When they can't fully relax, they cause pain, tenderness, and symptoms affecting sexual function, bladder, and bowel.
Why do pelvic floor muscles become hypertonic?
Common triggers include guarding in response to vulvar or vaginal pain, surgery or trauma, endometriosis, chronic UTIs, childbirth, hormonal changes, and stress. Once chronically tight, the muscles restrict their own blood flow and accumulate lactic acid - creating a self-perpetuating pain cycle independent of the original trigger.
Should I be doing Kegels?
Not if your muscles are already hypertonic. Kegels strengthen and contract the pelvic floor. If your problem is chronic muscle tightness, adding more contraction makes the problem worse. A pelvic floor physical therapist can assess whether your muscles are too tight, too weak, or both - and build the right program for your specific pattern.
What is the difference between hypertonic pelvic floor and vaginismus?
Vaginismus specifically refers to the involuntary muscle contraction that makes penetration impossible. Hypertonic pelvic floor is broader - chronic muscle overactivity that creates pain with and without penetration. They overlap significantly and are often treated with the same approaches.
When is Botox used for pelvic floor?
Botox is used when pelvic floor physical therapy hasn't achieved sufficient muscle relaxation - typically after a fair course of dedicated PT with a qualified therapist. It temporarily blocks nerve signals to overactive muscles, providing a window of reduced muscle tone during which PT can achieve deeper release. Results typically last several months and the treatment can be repeated.

Tight pelvic floor muscles can be released. Sex can stop hurting.

Dr. Toubi's Beverly Hills practice offers expert evaluation and treatment of hypertonic pelvic floor dysfunction - including Botox injections for patients who need more than PT alone. Schedule a consultation for a comprehensive assessment.

Pelvic Ultrasound

Pelvic ultrasounds play a crucial role in aiding both conception and pregnancy prevention. They provide valuable insights into a woman’s reproductive health and menstrual cycle, helping individuals and couples make informed decisions about their fertility journey.

Understanding the Menstrual Cycle

A woman’s menstrual cycle is a complex, carefully orchestrated sequence of events. Tracking this cycle accurately is essential for those trying to conceive and for those looking to prevent pregnancy effectively. Pelvic ultrasounds offer a window into this intricate process, helping individuals and healthcare providers understand where they are in the cycle with precision.

Conception Assistance

For those trying to conceive, pelvic ultrasounds provide essential information:

  • Ovulation Timing: Ultrasound guidance allows for the accurate detection of ovulation, pinpointing the release of an egg from the ovary. Knowing the precise timing of ovulation is crucial for couples trying to maximize their chances of conception.
  • Follicle Monitoring: Ultrasound can track the development of ovarian follicles, which house maturing eggs. Monitoring follicular growth helps identify the optimal time for intercourse or fertility treatments like intrauterine insemination (IUI)
  • Uterine Health: A pelvic ultrasound can assess the health of the uterine lining and detect any abnormalities that might hinder implantation or pregnancy. Conditions like fibroids or polyps can be identified and addressed.
  • Detecting Ovarian Cysts: Ovarian cysts can sometimes interfere with conception. Ultrasound can detect the presence of cysts and guide their management.

Pregnancy Prevention

Pelvic ultrasounds are equally valuable for individuals and couples seeking effective pregnancy prevention methods:

  • Cycle Monitoring: Ultrasound helps monitor the menstrual cycle, ensuring that contraceptive methods, such as hormonal birth control, are administered at the right time.
  • Confirmation of Pregnancy: In cases where pregnancy prevention methods fail, ultrasound can confirm the presence of pregnancy, allowing for timely decisions regarding continuation or termination.
  • Assessment of Contraceptive Devices: Ultrasound can be used to check the position and effectiveness of intrauterine devices (IUDs) and other contraceptive implants.

Personalized Fertility Management

The use of pelvic ultrasounds in fertility management offers a personalized approach to conception and contraception. It empowers individuals and couples with accurate information about their reproductive health, enabling them to make informed choices about family planning.
